Are looking to change which YouTube account is connected to your Google+ profile?
Well, it will require some juggling but we will be able to get your profile attached to the channel you want.
It will also have the same custom URL as your Google+ account if you have chosen it.
To do this, you will need to disconnect the existing YouTube channel from your current account. It will need to be attached to a Google Page.

Then you will need to contact YouTube support here to have the channel you want to connect to your Google+ profile:

When it has been connected it will automatically have the same name as your Google+ account.
Unfortunately there is no 'merge' option on channels, so any other ones will have to be retired.
